1) That I have successfully completed at least a basic course in SCUBA diving as given by a nationally
organized teaching and certifying organization.

2) I understand my participation in activities of the SunCoast Reef Rovers, Inc. is completely on a voluntary
basis.

3) While understanding the voluntary nature of the activities, I understand there are certain risks involved,
and I will accept these risks by participating in these activities.

4) I will not engage in unsafe behavior while diving or spear fishing. If I have to forfeit my diving rights,
 a refund will not be given.

5) I, for myself, my heirs, legal representatives, and assigns, do hereby release the SunCoast Reef Rovers,
Inc. from all suits, claims or demands of any kind which may result from death, injury, loss or damage
of any kind arising out or in connections with my participation in authorized Club activities.

6) I agree to provide a photo copy of my certification card and dive insurance card to the SunCoast Reef
Rovers, Inc.

7) I warrant that I am of legal age.

8) I warrant that I have read, and that I understand the above statements.

Refusal to sign this release disqualifies the individual from authorized participation in water related SunCoast
Reef Rovers, Inc. activities.
